The racist murder of Zahid Mubarek in March 2000 triggered the first ever Public Judicial Inquiry into a death in British prisons. What has happened to the recommendations made by the subsequent Keith Inquiry?  Has the prison service changed in its response to racism and violence within the prison estate? What can be done in the future?
